Primrose Communities has been offering specialized residential, educational, and vocational services for individuals diagnosed with Prader-Willi Syndrome since April 2018. Additionally, our team brings over 30 years of direct experience working with individuals who have developmental disabilities and are medically fragile.

Referrals and inquiries are often initiated by parents or guardians of individuals diagnosed with PWS. Formal referrals can also be made by county and state Departments of Health and Social Services or state/local Education Agencies, which typically provide the funding for placement. Primrose Communities accepts admissions from across the United States, and our team has extensive experience accommodating numerous out-of-state residents.

Currently, our homes and affiliates are located in Snellville, Georgia, Jackson, Michigan, Rives Junction, Michigan, and neighboring townships. Each community offers a comfortable and therapeutic residential living environment with specialized support systems tailored to each individual.

The length of placement varies based on each individual's needs. Due to the lifelong support required for those with PWS, placements are typically long-term; however, many individuals have successfully transitioned to less restrictive settings.

Each home develops a monthly activity calendar featuring a wide range of both community and in-house activities. These activities consider the interests of the residents, the affordability of the options, and the appropriate community integration skills of the individuals involved. Activities are planned daily, and residents are encouraged to participate in both planning and engaging in these activities.

Yes, due to the nature of the population we serve, the kitchen is kept locked, and all food is stored out of residents' sight and access except during scheduled meal or snack times. Additionally, employees are not permitted to bring food onto the premises. Environmental controls, such as locked refrigerators, freezers, and food cupboards, are used to ensure the health, safety, and well-being of each person, which is a critical aspect of our programming.

All individuals are evaluated by a registered dietitian who establishes a weight management plan. The dietitian consults with the group home support staff monthly to review and, if necessary, modify each individual’s plan. They also review house menus to ensure a wide variety of healthy and nutritious meals are being served.

Exercise is a vital part of each person's weight management plan. Group and/or individual exercise programs are developed for each resident, incorporating input from both the residents and group home staff. Exercise is integrated into the daily routine at Primrose Communities, with options including walking in the neighborhood or community, hiking in local parks, riding bikes, using treadmills or other exercise equipment, following exercise videos, dancing, and participating in active games. Residents also have the opportunity to take part in various Special Olympic sports. Additionally, Primrose Communities features a fully equipped onsite Fit Gym.

With assistance from our Care Coordinator, residents may opt to receive services from our consulting providers or choose from various medical professionals within the community. All selected medical professionals receive educational opportunities about PWS during orientation. Primrose Communities also has licensed clinical staff available for consultation 24 hours a day.
